Data from The National Longitudinal Study of Adolescent Health is available on the CSDE Windows system.
We have two versions of the data:
1. Public-use data set
The public-use data contains information on Add Health's nationally representative sample of adolescents. This dataset includes Wave I and II respondents and consists of one-half of the core sample, chosen at random, and one-half of the oversample of African-American adolescents with a parent who has a college degree. The total number of respondents in this dataset is approximately 6,500.
To apply for access to the public-use Add Health data set, please complete the CSDE Dataset Access Request form.
2. Restricted-use contractual data set
The restricted-use contractual data set is available to student and faculty affiliates
of CSDE. If you are a student doing your own research (i.e., not working on
a faculty affiliate’s research), this research needs to be part of your masters
thesis or dissertation. Under special circumstances, non-affiliates may access
the data. For more information about this, contact Cori Mar at cmmar@u.washington.edu.
Our restricted-use contract requires researchers to receive University of Washington IRB approval for using these data. In order to get this approval, you need to fill out a Modification Form. We have streamlined the Human Subjects Division (HSD) form so that it is easier to fill out. Click here to download our streamlined Modification Form. However, HSD periodically updates its forms, so it is possible that our form is out of date. Please check the HSD website to find the date of the current form and compare it with ours. If ours is out of date, please email us. Otherwise, fill out the form we have provided:
- Fill in Section A 1 box
- Fill in Table C (under Section I): New Researcher/Staff #1 – all of the boxes above "1. Does ..."
- If you are a student, similarly fill in Table C New Researcher/Staff#2 with your faculty sponsor (advisor) information.
- If you are adding less than two researchers then delete the second Table C. If you are adding more than two researchers then add another Table C. For instructions on how to do this, go to http://www.washington.edu/research/hsd/formRetrieve.php?ver=accreditation&id=187
- Send an electronic copy of the filled in streamlined Modification Form to cmmar@u.washington.edu
- If you are a student, please put two paper copies of your thesis or dissertation proposal in my mailbox in Raitt 206 or mail them to me Cori Mar, CSDE, Box 353412

CSDE will submit this modification to Human Subjects. Review at Human Subjects can two weeks or longer. When the modification is approved, we will notify you by email. At that time, we will need you to submit the Add Health Security Requirements form.
Print a physical copy, use a pen to complete the form, and return it to:

If you are a student doing your own research, you need to have your faculty sponsor (advisor) also fill out Attachments C and D. After Attachments C and D have been faxed to the Carolina Population Center, you will receive your Add Health user name as well as instructions for accessing the data by email.
